<p><span style="font-size:medium;">依據：無線寬頻接取業務管理規則第18條第3項辦理。</span></p>
<p><span style="font-size:medium;">公告事項：</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">一、申請人應於本會公告無線寬頻接取業務(以下簡稱本業務)申請期限內，依無線寬頻接取業務管理規則(以下簡稱本規則)第18條之規定，檢具下列文件及電子檔之唯讀光碟片各7份【但第(三)款至第(六)款文件各1份】，向國家通訊傳播委員會(以下簡稱本會)提出申請：</span></p>
<p style="margin-left:4em;text-indent:-2em;"><span style="font-size:medium;">(一) 申請書(格式如附件1)。</span></p>
<p style="margin-left:4em;text-indent:-2em;"><span style="font-size:medium;">(二) 事業計畫書及其附件(格式如附件2至附件6)。</span></p>
<p style="margin-left:4em;text-indent:-2em;"><span style="font-size:medium;">(三) 得標意願優先順序表(格式如附件7)</span></p>
<p style="margin-left:4em;text-indent:-2em;"><span style="font-size:medium;">(四) 報價單(格式如附件8)</span></p>
<p style="margin-left:4em;text-indent:-2em;"><span style="font-size:medium;">(五) 押標金匯款單回執聯影本。</span></p>
<p style="margin-left:4em;text-indent:-2em;"><span style="font-size:medium;">(六) 審查費匯款單回執聯影本。</span></p>
<p style="margin-left: 2em;"><span style="font-size:medium;">申請人繳交押標金與審查費，應分別以電匯方式匯入本會指定帳戶，匯款時務必填寫匯款人及申請人名稱、地址及電話；申請人應於提出申請時，對其所欲參與競價標的提出得標意願優先順序表，並提出對應之報價數值，其報價數值不得小於1.5％，報價以0.01％為單位；該得標意願優先順序表及報價單分開裝封，於封口處加蓋公司章及負責人章並密封遞送(信封格式如附件9)。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">二、事業計畫書應載明事項，其中從屬公司之關係報告書及控制公司之合併營業報告書，應依財政部證券暨期貨管理委員會發布之「關係企業合併營業報告書關係企業合併財務報表及關係報告書編製準則」之規定，編製截至95年12月底之資料，並記載下列事項：</span></p>
<p style="margin-left:4em;text-indent:-2em;"><span style="font-size:medium;">(一) 關係報告書：應包括控制公司之名稱、控制之原因、持股情形及其他對從屬公司財務、業務有重大影響之事項。</span></p>
<p style="margin-left:4em;text-indent:-2em;"><span style="font-size:medium;">(二) 合併營業報告書：</span></p>
<ol class="withNumber" style="margin-left: 4em;">
<li><span style="font-size:medium;">關係企業組織圖：列明各關係企業之名稱，並加註下列公司間之持股或出資比例：</span></li>
</ol>
<p style="margin-left: 8em; text-indent: -2em;"><span style="font-size:medium;">(1) 控制公司與從屬公司。(指有本規則第16條第2項第1款至第3款之關係者)。</span></p>
<p style="margin-left: 8em; text-indent: -2em;"><span style="font-size:medium;">(2) 從屬公司與從屬公司。</span></p>
<ol class="withNumber" start="2" style="margin-left: 4em;">
<li><span style="font-size:medium;">具有本規則第16條第2項第2款或第3款之關係者，應列明相同之名稱及合計持股或出資比例。</span></li>
</ol>
<p style="margin-left:4em;text-indent:-2em;"><span style="font-size:medium;">(三) 對於同一申請人及聯合申請人所定事項無不實陳述或虛偽記載切結書(格式如附件10)。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">三、申請人如有依證券發行人財務報告編製準則或商業會計處理準則等規定提具財務報告資料經驗者，請一併提供歷史檔案，以為審查財務規劃能力之參考。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">四、各項申請文件及書表，應以A4紙張直式橫書(圖表不在此限)。並應以中文書寫；使用外文證明文件者，應併提出中譯本。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">五、申請人所檢送之電子檔案，其事業計畫書及附件應以Microsoft Word 2000或2003 中文版本製作，其中附件2至附件5須以Microsoft Excel2000或2003中文版本製作，若非以前述格式製作者，申請人應無償提供合法版權之解讀軟體及其安裝、操作之說明各7份，並確保能在Windows XP環境下讀取。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">六、各項文件及其附件或附表以正本提出者，應於各文件之首頁蓋公司章及代表人章；若以影本提出者，應於該影本首頁蓋公司章、代表人章並註明與正本相符。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">七、事業計畫書內有關之證明文件、持股百分之一以上股東名簿、董監事名單等應分別裝釘成冊，並以附件方式提出。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">八、文件書表除申請書外，應裝釘成冊並裝箱，裝箱之最大尺寸規格分別為長42公分、寬37公分、高32公分或臺灣郵政公司之3號紙箱，且應於箱外長邊左上冊標示箱內所裝文件之份數編號、箱號及文件清單(格式如附件11)，正本應置於第1份箱內並於箱外載明正本字樣。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">九、申請人於申請後，應依本會通知，提出有關同一申請人及聯合申請人之關係切結書(格式如附件12、附件13)。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">十、擬申請人及其授權代理人得申請參與本會定於96年5月4日(星期五)下午2時假本會濟南路辦公室7樓會議室（地址：台北市濟南路二段14號）舉辦第1次競價流程說明會 (報名表如附件14)。</span></p>
