<p><span style="font-size:medium;">依據：電信法第42條第2項授權訂定之電信終端設備審驗辦法第25條。</span></p>
<p><span style="font-size:medium;">公告事項：</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">一、電信終端設備審驗合格標籤式樣說明，如附件四。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">二、販賣用電信終端設備型式認證申請書，如附表一。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">三、自用電信終端設備審驗申請書，如附表二。</span></p>
<p style="margin-left: 2em; text-indent: -2em;"><span style="font-size:medium;">四、電信終端設備符合性聲明申請書，如附表三。</span></p>
