<p>主旨：公告電信終端設備審驗管理辦法所定申請作業流程及書表格式，自中華民國一百零九年七月一日生效。</p>
<p>依據：電信管理法第四十四條第三項授權訂定之電信終端設備審驗管理辦法第二十五條規定。</p>
<p>公告事項：</p>
<p style="margin-left:4em;text-indent:-2em;">一、附件一「電信終端設備型式認證申請作業流程」。</p>
<p style="margin-left:4em;text-indent:-2em;">二、附件二「電信終端設備符合性聲明申請作業流程」。</p>
<p style="margin-left:4em;text-indent:-2em;">三、附件三「自用電信終端設備審驗申請作業流程」。 </p>
<p style="margin-left:4em;text-indent:-2em;">四、附件四「同意他人使用審驗合格標籤或符合性聲明標籤作業流程」。</p>
<p style="margin-left:4em;text-indent:-2em;">五、附件五「審驗合格標籤或符合性聲明標籤式樣」。 </p>
